Sight setup problem

I have a Pedersoli Rolling Block in 40-65. The problem is that the rifle shoots 12 inches left at 100 yards with the sights centered on the bore. This occurs with both barrel sights or tang sights. In order to shoot to point of aim, the sights must be adjusted way right or way off of being true to the bore.

Are there any suggestions as to what might be the problem or what to look for or measure.

If you have a good caliper ,I would measure from the bore to the out side of the barrel on both sides and see if there even close .If its off that much you could probably see the bore is off center with just eye balling it . You need to move the front sight .120 to get it to zero .
